Physician visits are the patient's responsibility and are covered by most insurance plans if indicated. Urobac is now covered by many insurance plans as a compounded prescription. You must first pay for your Urobac, then file a claim with your insurance company for reimbursement. A "letter of medical necessity" from your doctor sent with your claim is usually helpful and suggested to speed reimbursement.
